Police in Derry failed to investigate an assault by a killer who went on to leave a Donegal man permanently brain damaged in an attack in which a father-of-one was killed.

Paul Mythen (41) from Letterkenny received serious head injuries when he was attacked by Paul Greatbanks at Bayview Terrace in Derry in February 2011. Earlier this week Greatbanks pleaded guilty to murdering Patrick Harkin in the attack and also admitted the attempted murder of Mr Mythen.

Four police officers have been disciplined after an investigation by the Police Ombudsman in the North found they failed to investigate an assault by Greatbanks two months before the attack that killed Mr Harkin and left Mr. Mythen critically injured.
